CONSENT, WAIVER AND RELEASE FROM LIABILITY

Before each game all participants or guardians must sign a waiver. Each player 18 and under needs to have a parent/guardian sign this waiver. The waiver is available here in pdf format so that it can be printed, signed and presented at the game. Waivers will also be available at every event.
